During the transition planning process, you will work with your Circle of Support or transition team to set goals and make a plan for meeting those goals. Those same people can also help you prepare for college or vocational training. You should follow up with these people after you make the initial plan. Once you go to college or enter a vocational program, your school may offer guidance counselors to help you adjust to your new life. Ask your transition center, placement agency, transitional housing program, PAL staff, transition support case manager, and school staff for help finding other mentoring options in your community.
